Adecco Healthcare & LifeSciences is looking for a self-motivated, detail-oriented, organized, and tech-savvy Case Management Coordinator to support members throughout the Hialeah area of Miami-Dade County, FL.
In this role, you will conduct telephonic and face-to-face case management visits to assess, plan, coordinate, and support member healthcare needs while promoting overall wellness and quality outcomes.
Key Responsibilities:
• Conduct member assessments and coordinate individualized care plans
• Manage supportive and medically complex caseloads
• Coordinate care with PCPs, specialists, and healthcare providers
• Assist with prior authorizations and medication reviews
• Connect members with community resources and support services
• Monitor short- and long-term health outcomes
• Maintain accurate documentation and case management records
• Provide condition management education and wellness support
Ideal Candidate:
• Strong case management and care coordination experience
• Excellent communication and organizational skills
• Comfortable working independently in the field
• Tech-savvy with strong documentation skills
• Bilingual English/Spanish required
• Reliable transportation required for local travel

Pay Details: $36.07 per hour
